Congenital heart defects are often associated with infancy or early childhood. Yet for some individuals, structural heart conditions remain undetected until adulthood , sometimes discovered only after symptoms begin to interfere with daily life.

In community settings, symptoms such as persistent fatigue, shortness of breath, dizziness, or reduced stamina are frequently normalized or attributed to stress, aging, or lifestyle factors. Without structured educational frameworks, opportunities for early recognition and referral may be missed.

Extension and community health professionals play a critical role in advancing health literacy. However, few standardized, research-based tools exist to support consistent education around adult congenital heart awareness and symptom recognition.

As a result, conversations around heart health may lack the specificity and structure needed to support early intervention and informed self-advocacy.

To address this gap, the Rhythm of the Heart Certification & Licensing Program was developed to translate doctoral research into practical, community-ready education.

License Terms Overview:

1. Single Facilitator License

Each certification purchase provides a non-transferable license for one facilitator to deliver the Rhythm of the Heart curriculum within their organization or community.

This license is issued to the individual participant and may not be shared with additional facilitators.

2. Authorized Use

Certified facilitators may use the curriculum to:

• Deliver educational workshops or community presentations
• Facilitate the Rhythm of the Heart program in community settings
• Use approved materials included in the facilitator toolkit

• Certification licenses are issued to individuals rather than organizations and may not

be transferred between staff members without written approval from Uniquely Defined, LLC.

3. Prohibited Uses

The following actions are not permitted under the license:

• Reproducing or redistributing curriculum materials outside the licensed program
• Uploading materials to public or internal databases for unrestricted access
• Selling, sublicensing, or modifying the curriculum for commercial use
• Allowing unlicensed facilitators to teach the program

• Uploading curriculum materials to shared drives, internal learning management systems, cloud storage platforms, or databases where unlicensed individuals may access them.

4. Institutional Purchases

Organizations purchasing certifications for multiple facilitators must purchase one license per facilitator. Group certification packages may be available upon request.

Certification licenses are issued to individuals rather than organizations and may not be transferred between staff members without written approval from Uniquely Defined, LLC.

5. Duration of License

Each certification includes a one-year facilitator license beginning on the date of certification. License renewal ensures facilitators maintain access to updated materials and program support.

6. Intellectual Property

The Rhythm of the Heart curriculum, training materials, and program framework are the exclusive intellectual property of Uniquely Defined, LLC.

7. Agreement to Terms

By purchasing certification access, participants acknowledge and agree to comply with the Rhythm of the Heart licensing terms.

Wellness Elevated is operated by Uniquely Defined, LLC and provides independent educational programming.

Yes. The Rhythm of the Heart Certification & Licensing Program is available to qualified professionals and organizations seeking to implement structured heart health education within their communities. Individual certification and institutional cohort options are available.

2️⃣ What does the per-agent license include?

Each certification includes:

3️⃣ How long is the license valid?

The professional license is valid for one year from the date of certification. Renewal options are available to ensure continued access to updated materials and ongoing implementation resources.

4️⃣ Can agents share materials with colleagues?

No. The license is issued per individual agent and is non-transferable. Redistribution, duplication, or resale of curriculum materials is prohibited. Additional agents must obtain their own certification license.

5️⃣ Is this considered an institutional contract or individual certification?

The program is structured as an individual professional certification license. Organizations may purchase multiple certifications for agents; however, each license remains individually issued to the certified professional.

Program Developer & Certification Lead

Dr. Jacklyn Sanders is an Assistant Professor and Extension Specialist whose work focuses on advancing heart health education and increasing awareness of congenital heart defects diagnosed in adulthood. Her doctoral research examined the lived experiences of adults who received late diagnoses, identifying critical gaps in symptom recognition and public awareness.

With experience translating research into structured community programming, Dr. Sanders developed the Rhythm of the Heart curriculum to provide professionals with a scalable, implementation-ready framework. Her work centers on building research-grounded systems that strengthen health literacy and support compliant, institutionally aligned programming.
